Role Overview
This is a multifaceted Case Manager role involving:
Discharge planning
Utilization review
Swing bed coordination
You’ll be responsible for ensuring smooth transitions from acute care to post-acute settings, managing utilization, and overseeing swing bed services.
Requirements:
Experience:
Minimum 3 years in case management or utilization management
OR
5 years as an RN in an acute care hospital setting
Education:
Bachelor’s degree in a healthcare-related field (e.g., Nursing, Social Work, Healthcare Administration)
Licensure/Certification:
Valid healthcare license or certification (RN, SW, BSW, or equivalent)
*Offered pay rate will be based on education, experience, and healthcare credentials.
