Role Overview
This RN role focuses on providing skilled nursing care in patients' homes across all age groups. Responsibilities include:
Assessing patient needs (physical, psychosocial, environmental)
Developing and evaluating Plans of Care (POC)
Coordinating with interdisciplinary teams
Educating patients/families
Supervising HHAs/STNAs/LVNs
Managing pain and referrals
Ensuring compliance with third-party payers
Participating in on-call duties and team meetings
Required Skills & Experience
Software: Homecare Homebase – Required
Documentation: OASIS – Required
Licensure: Current RN license – Required
Certifications: CPR – Required; CHPN – Preferred
Driving: Valid driver’s license, reliable transportation, and insurance – Required
Experience:
1–2 years as RN in clinical care – Required
2+ years in home health – Required
Education:
Required: Associate Degree in Nursing or Nursing Diploma
Preferred: Bachelor of Science in Nursing (BSN)
*Offered pay rate will be based on education, experience, and healthcare credentials.
